Medellín or Cartagena, which one should you visit first?

Choose Cartagena for beaches, colonial streets and a short celebration trip. Choose Medellín for mild mountain weather, a bigger city, better value and a scene you settle into for a week. If you have seven days or more, do both: four nights in Medellín, three in Cartagena, one short flight between them.
Weather
Medellín sits at 1,500 meters and holds around 22 to 28 degrees Celsius all year, hence the nickname city of eternal spring. Rain comes in short afternoon bursts.
Cartagena is sea level Caribbean: hot and humid every single day, usually 30 to 33 degrees with real humidity behind it. Lovely at 7am and at 7pm, punishing at 1pm.
The feel of each place
Medellín is a real city of two and a half million people. Neighborhoods, gyms, coworking, an enormous food scene, mountains on both sides. You can live here for a month and keep finding things.
Cartagena is a small, beautiful, hot postcard. Three or four days of pure atmosphere, then the water takes over. It is a place you visit rather than a place you settle into.
Nightlife
Both are excellent, in different registers. Medellín has more of everything: rooftops, clubs, live music, late nights and a crowd that goes out midweek.
Cartagena is more concentrated. Getsemaní bars, a handful of clubs, the walled city buzzing until 2am, and the party often ends up on a boat or at a villa instead of a venue.
Cost
Medellín is the better value on almost every line: accommodation, food, drivers, everything.
Cartagena carries a tourist premium, particularly in the walled city and for boats. It is still reasonable by North American standards, just noticeably higher than the rest of Colombia.
- Stays: Medellín cheaper by roughly a third for equivalent quality.
- Dining: similar at the top end, Medellín far cheaper in the middle.
- Transport: Medellín cheaper, though Cartagena distances are shorter.
- Activities: Cartagena costs more because the good ones involve boats.
Safety
Both are safe with normal precautions. Cartagena's tourist core is small, busy and heavily patrolled, which makes it feel easier at night. Medellín is larger, so the rules about neighborhoods and transport matter more.

How to combine them
The flight between the two is about an hour and runs several times a day. The sequence that works best is Medellín first, Cartagena second: acclimatize in the mountains, then finish on the coast so the last days are beach days.
